In watching the promos for this ep, they talk of a hideous monster (or something-or-other) in South Park and show Chef dragging off a reluctant Stan (or is it Kyle) with what looks to be blood around his mouth.
Theory: The hideous monster is the school nurse.

My theory: Kyle is the monster (unwillingly, of course), and Kenny must fight/kill him to save the school (like a mirror-image of Pink Eye, when Kyle was forced to kill Kenny to save the town).
