Biologically it is, but people are more than just a biological organism, people have an extremely complex psyche that is not fully understood even by modern psychology. If someone feels that they are, say, female while biologically they are male, and then they have a sex change, to say that they are now a male with mutilated genitals dressed in drag is a very narrow viewpoint.
