Remember Mrs. Bouvier. She only appeared in about 2 episodes, and we have never really seen her after that.
I think they choose not to make her as a character because Julie Kavner, who does the voice of Marge, Patty, and Selma probably thought that those 4 would sound the same, being that they're all related.
And since Mrs. Bouvier looked really old, we can probably say that she died, but the show never mentioned it.
