Where Homer crosses over into our world. I think there's some sort of weird gateway/portal thingy in the wall of his living room and he goes through it and ends up in the real world. At the end of the show you see a shot of a cartoon Homer very nervously walking down a normal street, but he bucks up a bit when he sees we have cakes here as well.

Can't remember the exact details leading up to the quote but I'm imagining it was something along these lines....

I think someone had died and maybe Marge was trying to gloss over it for the kids, e.g. gone to a better place, happier where he is now etc. and made it sound so good that Homer said very wistfully and longingly "I wish I was dead"

The episode you're referring to is "Bart the Fink." When they thought that Krusty was dead, Homer was trying to comfort Bart by telling him all the people Krusty would meet in heaven.
