In "Co-Dependent's Day", when Barney tells Marge he's been in rehab five times, he demonstrates it by holding up five fingers. Of course this takes two hands, showing all four fingers on one hand and one finger on the other. Is this an old gag or new?

I think the first reference was when Homer counted all the words in "Greasy Joe's Bottemless Barbeque Pitt" and had to use a finger from his second hand.

Yeah, I think that might have been the first specific reference to it. While the characters have always had four fingers on each hand, first season episodes tended to ignore this fact, as seen in the reference to "five-finger discount" in "The Telltale Head" (a later episode, possibly "Marge Be Not Proud," mentioned "four-finger discount"), and Homer saying, "Here's five you haven't met!" when punching Barney in "No Disgrace Like Home."
