Was Mendoza the guy who offered McBain a poisonous cream puff? or was that someone else? If it was someone else, have we ever seen Mendoza and how many times has he been refereed to?

Yes, it was Mendoza. Mendoza also appears in 7F14(or 16?) where he's thrown from the window into an exploding tanker truck.
