If you add a commercial *anywhere*, you need to extend the show. Either start it one minute earlier, or end it one minute later. There's no way to add a commercial in the middle and still have the show run 30 minutes.

While I'm not sure that there's still any legal limit on how much commercial matter can be shown in a program, Wiseguy's explanation is correct. Shows start a minute early or end a minute late so that the network can get one more commercial into a show that can charge a higher rate without having to cut the show.
