Somebody raised this issue recently, Channel 4 in the UK screened 'A Streetcar Named Marge ' two days ago! Not exactly inspired scheduling, but they were probably unaware.
It was on at 6pm whilst the other channels were running their evening news, obviously packed full of the latest news.

I didn't see it, but I'm told that yesterday, the US game show "The Price is Right" aired a rerun in which one of the showcase prizes was a trip to New Orleans.
The folks at CBS obviously spotted their error (or received a bunch of complaints), as it was later reported that the feed for the west coast (two hours after the airing on the east coast) aired a different rerun.
I heard that CBS screened the upcoming reruns to avoid such references, and missed it. Seems awfully hard to miss. "Win a trip to New Orleans" isn't exactly a subtle reference. What were they looking for? Maybe just checking that none of the contestants were from New Orleans (and currently missing), I suppose.

Maybe they were just zipping through the tapes really fast. When the prize is a trip to somewhere, they show a picture of the place for a few seconds. Someone fast-forwarding through the tapes, if they weren't attentive enough, might miss this.
I also heard (via "Wait, Wait, Don't Tell Me") that another prize in the same prize showcase was a speedboat.
