So I can get basically every other sexual reference in the show except for this one in "The Rye" it really confuses me. Elaine says her musician friend "doesn't like to do -everything-".
It really confuses me when Jerry adds "Why not? You're there." I honestly have no idea what it's referring to.

And I still don't understand how Jerry's "why not? you're there" comment fits into it-- wouldn't -have- to go there to do something?
The only thing that really made me think it was oral sex to begin with was the bad sax playing at the end of the episode. Everything else seems to vague that it's hard to tell what's being talked about.
