I think Jerry's girlfriend refused the pie because he'd eaten off the fork and she was too finicky to share it. This is particularily ironic as her father was a real slob (ie washroom/pizza incident).
Yeah. The way she was looking at the plate and fork as he was offering her the pie suggests this. At the time, I thought that her emphatic "I've had *just* enough to eat" (or something like that) might have been the reason, but the subsequently revealed theme of people being repulsed by gross things others have done (I hope that spatula was melted down and disposed of at sea) makes me go with the "saliva on the fork" theory.
The way she looked at the plate and fork does suggest her disgust with sharing the pie with his saliva, but I did not make much of it, because that seems to be a perfectly normal thing to do on a date or just with any friend who wants a taste. This is also the way that Jerry reacted to her refusal.
I thought so too. Or, perhaps she wanted her own piece of pie. Maybe she is not into sharing food from the same plate. It's not quite as appetizing after he had taken a few bites of it. He should have offered her the first bite if he wanted to share (IMHO).
Given the slob level in her father's restaurant, it does not surprise me she did not want to eat restaurant apple pie when Jerry offered her some. The mystery is why she ate it when Kramer saw her there the next day.

She wouldn't eat the pie in his presence because they needed some gimmick for the plot. This gimmick is all too reminescent of the masseuse who wouldn't give him a massage but did it for Kramer. The running gimmick is that there is always something that makes Jerry break up with all these women.
It's the great enigma.
