episode title questions

i've got a question about episode titles, that pertains to any tv show in general, but since i'm here and i just read about "the betrayal", i'll ask here.

i've never noticed episode titles in shows that don't add them at the beginning of the episode, so i'm assuming that unless they're hidden in the credits somewhere (like the episode number, like 0FOA or however they do it), i don't know where they come from. can anyone fill me in? not that it's a big deal to me; i'd just like to know.
