Maybe it was before that Kramer used to shower. I remember him floating in his inflatable bath: you know the one where he froze to the bone once. :) But I don't remember the time periods portrayed in any of those episodes, so can't really say for sure.

Because they needed shower heads for the joke. That's the "situation" part of "situation comedy".
Not all of the episodes are written by the same people...
I can't remember which episode it is (The Keys?), but there is an occasion where Jerry comes home to find Kramer taking a bubble bath in his tub. He's wearing a shower cap, using a scrub brush, the whole bit. So he did take a bath at least once!
