I just noticed this the other day as I was re-watching the heart attack. Kramer is telling George about the Holistic healer. In an effort to steer George away from the traditional hospital, Kramer says his friend bob sacamano went in for a "routine" hernia operation and now just sits in front of the window all day saying "hello, my name is bob!" This is very inconsistant with other stories we have heard about bob such as the rat hat he got for george in " The Chicken Roaster."

The joke was that Bob went into he hospital for a hernia operation, but they obviously screwed up and removed, uh, other organs. This is why Kramer tells George to use the holistic healer instead. I'm sure you've seen enough comedy to understand what's implied when someone's voice becomes higher pitched. So, Kramer's buddy Bob's surgeon removed his gonads by mistake, and Bob speaks in a falsetto voice but how could this possibly prevent him from selling hats in the park??
Oh, maybe you're hung up on the 'sits in front of the window' comment? I think this simply meant that Bob lost his 'urges' and became lethargic when there was no longer any testosterone in his body. Ever seen a dog after he's been 'fixed'? He also loses interest in things he used to like doing.
