Anybody notice this? Maybe I'm wrong. When George dipped the Corned beef sandwich in the mustard, the sandwich had no toothpick. When the sheets of the bed were lifted, the sandwich was sitting on the bed with a toothpick in it.

Oh, man. This is really stupid. I'm sorry. This is such a small and ridiculous thing to point out.
There are so many more important points of continuity that "should" be discussed regarding this program and not whether a toothpick was present or not.
Public transportation? Also I only watched the show live but I though Kramer only borrowed Jerry's car because his (Kramers) didn't have air conditioning to keep the blood cooled.
Kramer knew who pushed him, Elaine was talking to him at the door for a good amount of time trying to convince him to leave first and she only pushed him as the front door was opening becaue he wouldn't leave on his own.

One of the few things that bugs me about "Seinfeld" is the lack of attention paid to scene continuity. And you can bet that the cuts syndication makes doesn't help.
I taped the episode and upon re-watching it I paid attention to this. Turns out the toothpick was in there the whole time. As he's dipping, the side with the toothpick is not visible, however in the clip right before it is visible and you can clearly see the toothpick.

this happens all the time in tv and movies. it is really cheesy to those who have an eye for this.
basically it is called "poor continuity".
most likely the scene without the toothpick was filmed 3 to 4 hours prior to the scene with the toothpick. then someone would say, "shouldn't we put a toothpick in it" but didn't think about the scene prior to this. or because they do the take so many times they keep bringing new sandwiches in to george and one of them happened to have a toothpick in it. that is why those people only get $100 to to $150 a day for 12 - 14 hour shifts.

The inconsistency that I noticed with the 10/16 episode was that Jerry had knicked his jugular and therefore was wearing a bandage on his neck in the hospital. But, as soon as he is out of the hospital....he is not wearing a bandage anymore. Gee, that major blood letting injury healed fast!
