I believe that Kramer relates a bad childhood experience he had when he went to the circus somewhere in the history of the show. I think this is the root of his fear of clowns which is compounded when he is confronted by Crazy Joe Devola in his I, Paliacchi clown makeup. Kramer continues his bad experiences at circuses when he passes his kidney stone in a circus bathroom in The Gymnast.
