Shuck - I thought it was Nostrom
Kramer has used 2 forms of this alais- in his audition for a part in JERRY- he was Martin Van Nostrum & as the doctor he was Dr Van Nostrum. So I assume that Van Nostrum is the last name and we are not told the good doctor's first name.

Actually, according to the Entertaiment Weekly Seinfeld Companion, it's spelled Van Nostrand. And I think he actually used the name at least four times--once as Martin Van Nostrand when trying out for the Kramer role in the JERRY show and once as Peter Van Nostrand when trying to sneak into his mom's ex-boyfriend's place to get his jacket back. Then, he also used the name in his doctor role twice--once while checking out Kreuger and the other time when he was trying to get Elaine's chart from her doctor's office.

I thought it was "Nordstrand", which is a Swedish name (which means "North Beach" in english).
