May I just say that 'Art Vandalay' can be traced all the way back to the second or third epeisode of Seinfeld. I think it was called the Stakeout, where Jerry met a girl at a diner he went to with Elaine and he met a girl but forgot something about her (I'm not really sure) and Jerrya and George went to her workplace and stood in the foyer waiting for. They planned to say that Jerry was meeting George or someting like that. Anyway George wanted to pretend,like always, that he was an architect. He decided that he wanted use another name as well and that was when George first came up with Art Vandalay.

Is Dr. VanNostren the name Kramer used when he performed the examination on George's boss, probably others as well?

Van Nostrand is how it's spelled. He also used that last name in "The Nose Job", "The Pilot", and "The Package". I believe he used different first names, though.
