Very thorough answers but aren't you forgetting a couple of obvious examples?

Jerry's father was first played by a bald actor who was doing a "combover" as Elaine referred to it. This guy preceded the current Morty (is it Barry Martin?).

On the handicapped parking episode, Frank Costanza was first played by John Randolph. Curiously, this episode was partly reshot with Jerry Stiller for syndication.

As no post is complete w/o a little opinion, I think the first Lloyd Braun was much better. It's real easy to see why George hated him. I think they should have created a new character for the old movie house episode.
