I was at Yankee Stadium yesterday. I know that it was 95 degrees with nearly 100% humidity, but sitting 20 rows or so in front of me was a teenage boy wearing a blue Yankee warmup jersey with a big NY on the front and the name Costanza and numer 00 on the back.

I would think any good sporting goods store can do it..I dont know where you are,,but there is a good store on the grounds of MSG here in NYC that I am sure can do it.....look for a store that does custom work for uniforms..yellow pages...under uniforms maybe/tailoring/etc
or any tailor.
Or, any sports apparel catalog that sells jerseys will allow you to customize
the name and number for a few dollars more. I have a Dodgers jersey that
says "Ratboy" across the back.
