Elaine: Head to toe?

I just remembered something that I never "got."

There was that one episode where Elaine told Jerry that she and some other guy slept together on a couch. Jerry asked her if any "funny business" went on. Elaine said no because they slept "head to toe." Jerry said "So what, your genitals are still lined up." Elaine responded "No, I slept with my back to him." Jerry looked confused after that. I'm confused too. My response would be "So what, you could still do it doggy-style."

So what am I missing here, or was it just supposed to not make any sense?

He was thinking that she could still take it up the ass.
