yes they can understand him, but Lois usually ignores what he's actually saying. It's usually some like: Stewie: "Blast, you vile woman!" Lois: "Aw, is poor Stewie upset?"

They have answered him directly before, so they can understand him, they just usually write him off as a baby, while we see him as having the mind of an adult.

I think, like Jon & Garfield, the family doesn't understand him literally, but they get the general sentiment he's expressing.

Everyone understands Stewie AND Brian. Whats funny is that it doesn't faze them in the least that there is a toddler talking like a 50 year old megalomaniac, or a smooth talking Dog with more brains than the whole lot of them sitting in the front room.

Lois just ignores Stewie's rantings just like any other Mother ignores what their baby is trying to say to them when they whine INCESSANTLY.
